In order to render services:
We will need:
1) For the client to be 18 years of age or older.
2) Must bring current State ID or Passport.
(School ID w/ Birth Certificate is acceptable)
Since we offer Body Piercing for Minors,
we need:
1) Parent/Legal court appointed guardian to be present w/ their ID
2) State/School ID or Passport
3) Birth Certificate - copies/pictures are accepted but must not be blurry, cropped or cut to be accepted.
There are NO EXCEPTIONS.
We will not provide services without them.
If you choose to lie or provide false information on any form/to any practitioner of this establishment you are liable for prosecution.
